As part of my clinical training during the dietetic internship at Cedars-Sinai Medical Center, I completed a detailed case study on the nutritional management of an 83-year-old patient with a small bowel obstruction (SBO) and multiple surgical complications, including peritonitis, acute kidney injury (AKI), and malnutrition. The report highlights critical nutrition support decisions—including initiation and adjustment of total parenteral nutrition (TPN), macronutrient recommendations, use of SMOFlipid, and protein dosing in the context of AKI. My analysis integrated current clinical guidelines and research to justify interventions and emphasized interdisciplinary collaboration to support patient recovery. This project deepened my clinical judgment in managing complex nutrition cases in critically ill patients.
